BRCA1 and BRCA2 Gene Mutations

Understanding BRCA Genes

BRCA1 (breast cancer gene 1) and BRCA2 (breast cancer gene 2) are tumor-suppressing genes that help repair damaged DNA and protect against certain cancers. When these genes function normally, they reduce your lifetime risk of breast and ovarian cancer. However, harmful mutations can interfere with DNA repair, leading to higher chances of developing cancer.

Inherited mutations of BRCA1 or BRCA2 account for about 3 percent of all breast cancers—roughly 7,500 women each year in the United States. Carrying a mutation does not mean you will develop cancer, but it does increase risk significantly.

The Difference Between BRCA1 and BRCA2

Although both genes are linked to hereditary breast and ovarian cancer, they differ slightly in frequency and risk profile.

BRCA1

  • More common

  • Higher risk when mutation occurs

  • 55–72% chance of developing breast cancer by age 70

  • Often linked to triple-negative breast cancer

BRCA2

  • Less common

  • Slightly lower risk when mutation occurs

  • 45–69% chance of developing breast cancer by age 70

  • Also associated with male breast and prostate cancers

Both BRCA1 and BRCA2 mutations can also increase the risk of ovarian, pancreatic, and melanoma cancers.

Genetic Testing and Counseling

Genetic testing for BRCA1 and BRCA2 is available through a simple blood or saliva test. You may benefit from testing if you have:

  • Multiple blood relatives with breast, ovarian, or prostate cancer

  • A known family BRCA mutation

  • Breast cancer diagnosed before age 50

  • Male breast cancer in your family

Testing should be guided by a certified genetic counselor to interpret results and discuss next steps. Most insurance plans cover BRCA testing, and assistance programs may be available for patients with financial barriers.

Preventive (Prophylactic) Mastectomy

Testing positive for a BRCA mutation can be emotionally challenging. One highly effective option to reduce risk is prophylactic mastectomy, the removal of at-risk breast tissue before cancer develops. Studies show that prophylactic mastectomy can lower breast cancer risk by up to 95 percent in BRCA-positive women.

While this surgery cannot guarantee complete prevention, it significantly reduces risk and allows early reconstructive planning. Breast reconstruction does not interfere with cancer detection or future treatments.

Reconstruction Options After Mastectomy

At Town Plastic Surgery, our surgeons specialize in advanced breast reconstruction for BRCA-positive patients, restoring natural form and symmetry after mastectomy. Each plan is personalized to your anatomy, goals, and oncologic recommendations.

Autologous (Flap) Reconstruction

Uses your own tissue—skin, fat, and sometimes muscle—from another area (abdomen, back, thigh, or buttocks) to recreate the breast. Common techniques include:

  • DIEP Flap

  • Free TRAM Flap

  • Pedicled TRAM Flap

  • Latissimus Dorsi Flap

  • Non-Abdominal Free Flaps

Implant-Based Reconstruction

A two-stage approach using tissue expanders followed by silicone or saline implants to achieve the desired shape and volume.

Hybrid (Implant + Flap) Reconstruction

Combines your own tissue with an implant for optimal contour, softness, and long-term stability—ideal for patients with limited tissue volume.

All reconstructive options are covered by law under New York State Insurance Law § 4303(x) and the Federal Women’s Health and Cancer Rights Act of 1998 (ERISA), ensuring comprehensive coverage for breast reconstruction after mastectomy.

Types Of Breast Reconstruction Surgery Procedures

Learn About Your Breast Reconstruction Surgery Options

  • Breast-conserving surgery, also known as a lumpectomy, removes the cancerous tissue while preserving as much of the breast as possible. Reconstruction after this procedure often involves reshaping the remaining breast tissue to restore a natural appearance. This may also include symmetrizing the other breast to achieve balance.

  • Implant-based reconstruction involves placing a saline or silicone implant to restore the breast's shape and volume. This procedure is often performed in stages, beginning with a tissue expander to create space for the implant. Once the tissue is ready, the permanent implant is placed, offering a relatively straightforward approach to breast reconstruction.

  • Flap procedures use tissue from another part of your body, such as the abdomen, back, or thighs, to reconstruct the breast. This method can provide a more natural look and feel compared to implants, but it is a more complex surgery. The most common flap procedures include the DIEP flap, TRAM flap, and Latissimus Dorsi flap, each named after the donor site.

  • Reconstruction of the nipple and areola is typically the final step in the breast reconstruction process. This can be achieved through various techniques, including local tissue rearrangement, skin grafting, or even tattooing to create a realistic appearance. While this step is optional, it can significantly enhance the aesthetic outcome of the reconstruction.

What You Need to Know

  • Breast reconstruction is a surgical procedure aimed at restoring the shape, appearance, and size of the breast following a mastectomy or lumpectomy. It can be performed using implants, autologous tissue (such as tissue from your abdomen or back), or a combination of both. The goal is to rebuild a breast that closely resembles your natural breast, helping you regain your confidence and sense of wholeness after breast cancer surgery.n text goes here

  • Choosing breast reconstruction is a deeply personal decision. Many women opt for reconstruction to regain their natural breast shape and feel more like themselves after a mastectomy. Beyond physical restoration, the procedure can also have significant emotional and psychological benefits, aiding in the healing process and improving overall quality of life.

  • Breast reconstruction offers numerous benefits, including the restoration of breast symmetry, the ability to wear a wider range of clothing comfortably, and an enhanced sense of body image and self-esteem. It can also provide a sense of closure for some women as they move forward after breast cancer treatment.

  • Most women who have undergone a mastectomy or lumpectomy are candidates for breast reconstruction. The timing of reconstruction can vary—some women opt for immediate reconstruction during the same surgery as their mastectomy, while others choose delayed reconstruction. Factors such as overall health, cancer treatment plans, and personal preferences play a role in determining the best approach for each patient.

  • During your consultation, Dr. Ratner will work closely with you to understand your individual needs and goals. Whether you're considering immediate reconstruction with your mastectomy or a delayed procedure, Dr. Ratner will collaborate with your breast surgeon to develop the best strategy for your reconstruction. He will carefully explain your options, considering factors such as your age, health, anatomy, and personal preferences. This is your opportunity to openly discuss your desired outcome, ensuring that the surgical plan aligns with your expectations. Dr. Ratner will also provide a comprehensive overview of the procedure, including potential risks and recovery details. Feel free to ask any questions you may have—this consultation is the foundation for a personalized approach to your breast reconstruction journey.

  • Breast reconstruction can be performed using several techniques, including implant-based reconstruction, flap reconstruction using your body’s own tissue, or a combination of both. The choice of technique depends on various factors, such as your body type, health status, and personal preferences. Dr. Ratner will work closely with you to determine the best option tailored to your needs.

  • Breast reconstruction surgery is typically covered by insurance, especially when it is part of the treatment plan following a mastectomy. However, coverage can vary, and it's important to check with your insurance provider for specific details regarding your plan. In some cases, additional costs may not be fully covered, and patients might need to pay out of pocket. At our practice, we understand the financial considerations and are committed to making your care as accessible as possible.

  • Like all surgeries, breast reconstruction carries some risks, but choosing an experienced, board-certified surgeon like Dr. Ratner significantly reduces the likelihood of complications. The most common risks include minor bleeding, infection, and temporary issues with incision healing, all of which are closely monitored and managed by our expert team. For flap reconstructions, there is a small chance of partial or complete loss of the flap, as well as temporary numbness in the donor or reconstruction site. If you opt for implant-based reconstruction, potential complications could include capsular contracture or implant rupture, both of which can be addressed if they occur. Importantly, breast reconstruction has no known impact on the recurrence of breast cancer and typically does not interfere with chemotherapy or radiation should further treatment be necessary. Dr. Ratner and his team are committed to ensuring your safety and comfort, guiding you through every step with care and expertise.

  • The transformation after breast reconstruction can be truly life-changing. Before the surgery, you may feel uncertain or self-conscious about your appearance following a mastectomy. Dr. Ratner understands these concerns and is dedicated to helping you regain your confidence. After the procedure, many patients find that their reconstructed breasts look natural and restore their sense of wholeness. The 'before and after' results can be remarkable, offering not just a physical transformation, but also an emotional and psychological boost. Dr. Ratner's skill and attention to detail ensure that your results are tailored to your body and your goals, helping you move forward with renewed confidence.

  • Recovery from breast reconstruction surgery varies depending on the type of procedure performed. Most patients can expect some discomfort, swelling, and bruising, which can be managed with pain medication. You’ll receive detailed instructions on caring for your surgical site, activity restrictions, and follow-up appointments to ensure a smooth recovery.

  • The results of breast reconstruction are long-lasting, with most women enjoying their new breast shape for many years. However, it’s important to understand that additional procedures may be necessary over time to maintain symmetry or address changes in the body. Dr. Ratner and his team are committed to providing ongoing care and support to ensure your results remain optimal.
